"Sarcomatoid carcinoma is a rare and aggressive form of squamous cell (SCC). It biphasic tumor that has been shown to be dedifferentiated monoclonal classic carcinoma. The factors contribute proliferation SCC sarcomatoid are not precisely known but some studies revealed the importance anterior radiation therapy. This case report describes 62-year-old man, heavy smoker alcohol drinker diagnosed with laryngeal neoplasm. pathological diagnosis was well-differentiated piriform sinus. Surgery performed pathology medium differentiated SCC, pT4aN1M0. patient underwent external radiotherapy (RT) 66Gy/tumour bed in 33 fractions. At 4 months post RT clinical exam recurrence near tracheal stoma. A second surgery sarcomatoid-looking areas, two surgical margins being invaded by tumor. Postoperative computer tomography (CT) neck showed no evidence macroscopic disease. next treatment used chemotherapy cycles carboplatin paclitaxel soon after, an inoperable locoregional he started Nivolumab followed rapid progression. Squamous dedifferentiation histology cancer, behavior requires intense follow-up."
